how to repair a ceiling with mold and chipping properly before painting again?

My bathroom ceiling has a little mold and is chipping. Do i need to scrap off all the existing paint as much as i can that is chipped and do i need to put some kind of cealinte or caulk before painting the whole ceiling? i want to make sure that after i paint, you cant tell where i chipped away. im afraid the ceiling will look uneven. Thanks!
